Chyllia Dixon, PMHNP
Psychiatric Nurse Practitioner
Chyllia Dixon is a Psychiatric Mental Health Nurse Practitioner based in Phoenix, Arizona. She is passionate about integrating holistic health modalities into her work with medically complex patients. Chyllia is a natural educator, sharing what she has learned through her own life experiences and from the variety of trainings she has attended. She is an editor and author of a best-selling psychiatric mental health nursing textbook and has authored many other publications, all with the aim of informing healthcare providers and transforming the patient experience.
Chyllia has worked her way up the ranks from nursing assistant to nurse practitioner, so she has a keen understanding of the various front-line roles within the healthcare system. She loves sharing her knowledge and has spent thousands of hours teaching, precepting, and presenting to a broad range of learners.
Chyllia’s unique accomplishments include holistic health training in massage therapy, hypnotherapy, aromatherapy, and other healing modalities. Her training in alternative healing practices involves more hours than her more formal training as a nurse and nurse practitioner. She uses both avenues of education to connect with a diverse population of patients and students and enjoys learning about and highlighting the different nuances of culture and it’s impact on mental health, physical health, and healthcare delivery.

My passion for helping others has been the driving force in both my professional and personal life. Whether I am leading a team or working on community projects, my focus is always on creating positive impact and fostering growth in those I serve.
At the heart of my leadership style is a deep sense of empathy. I believe that true leadership is about listening, understanding, and supporting others in achieving their potential. This belief has guided my approach, ensuring that my efforts are not only effective but meaningful.
Beyond my professional pursuits, my family is my cornerstone. They bring immense joy and strength to my life, serving as my motivation and grounding me through all of life’s challenges. My love extends to my pets, who add warmth and companionship to our home, reminding me of the simple joys and unconditional love that enrich life.

Sharon Miller, Administrative Assistant
Sharon Miller's journey is a testament to adaptability, compassion, and continuous growth. Hailing from Scottsdale, Arizona, she honed her skills as a hairstylist, color specialist, and salon manager for two fruitful decades before embarking on a new chapter in the healthcare field.
The onset of the COVID-19 pandemic marked a pivotal moment in Sharon's life as she transitioned into the role of caregiver. During this challenging period, she found immense joy and purpose in providing care for two elderly women, embodying empathy and selflessness in the face of adversity.
Amidst her professional endeavors, Sharon holds her role as a mother to her daughter as her most cherished accomplishment. This unwavering bond serves as a guiding light in her journey, shaping her values and priorities. In 2021, Sharon pursued her passion for healthcare by graduating from Pima Medical Institute with a degree in Healthcare Administration. This educational milestone opened doors to new opportunities, leading her to join Dr. Palpu Hazel's team as an Administrative Specialist in May 2021. Over time, her dedication and expertise propelled her into a full-time position, marking her commitment to making a difference in the healthcare sector.
As the winds of change swept through Dr. Hazel's practice, Sharon embraced her evolving role with grace and determination. With Dr. Hazel's retirement, the practice transitioned seamlessly under the guidance of Chyllia Dixon, PMHNP, at Hazelwood Mental Health. When not working, she loves spending time with her daughter and her puppy dog and spending time with friends.

Meet Jasmin Monira Labungan - our administrative assistant. She's had quite the career journey, from starting as a Registered Pharmacist to becoming a Customer Service Representative, and now a Medical Assistant. What makes Jasmin truly special is her dedication to treating clients and patients with integrity, eloquence, and professionalism. She knows that great communication leads to better outcomes, and she lives by that.
Jasmin was born Philippines. When she is not working for Hazelwood, in her free time, she loves cooking up a storm, binge-watching dramas/movies, and cuddling with her 6 adorable dogs.
