Want to make your WAIT TIME SHORTER when you come in for your appointment? Fill out your forms BEFORE you come in for your appointment. Take your time and fill out your forms on your computer or Phone by downloading forms here (you can type your information in because these are fillable forms!) and print them out. Please dont forget to bring your filled out forms to your appoinment.
