Hazelwood Mental Health is a Scottsdale-based mental health clinic offering a mix of psychiatry, therapy, and holistic care to individuals across the lifespan. We work with children, teens, adults, and elders in a welcoming and inclusive space.

Who are your providers?

Our team includes psychiatric nurse practitioners, therapists, and support staff who bring a range of clinical and lived experiences. Each provider is chosen for their warmth, skill, and commitment to compassionate care.

Do you offer holistic or integrative mental health care?

Yes. We incorporate mindfulness, movement, lifestyle medicine, and other integrative modalities when appropriate. Many of our clinicians have training in somatic, trauma-focused, and non-traditional therapies.

Services Offered
What types of mental health conditions do you treat?

We support clients with depression, anxiety, ADHD, trauma, mood disorders, OCD, autism, grief, life transitions, and more. If you would like to know if we treat a specific condition, please contact us to confirm. 

What services are available?

We offer psychiatric evaluation, medication management, individual therapy, family/couples therapy, group support, and referrals for additional services.

Do you provide therapy or only medication management?

Both! Some clients receive therapy, some medication management, and many benefit from both. We can help guide what’s right for you.

Are services available for both adults and children?

Yes. We serve clients from ages 5 to 95. We have clinicians who specialize in pediatric, adolescent, adult, and geriatric mental health.

Do you offer support for co-occurring disorders?

Absolutely. Many of our clients experience overlapping mental health and substance use concerns. We tailor care plans to meet complex needs.

Appointments and Scheduling
How do I book an appointment?

You can call us, use our online booking form, or email the front desk. Our team will walk you through the next steps.

Can I get help choosing the right service or provider?

Yes. Our intake team is happy to listen and help match you with a provider who fits your needs.

What is the wait time for new patients?

Wait times vary depending on provider and service type. We do our best to offer timely appointments and will let you know availability up front.

What is your cancellation or rescheduling policy?

We ask for at least 24 hours’ notice for changes or cancellations. Late cancellations may incur a fee. We understand life happens—just keep us in the loop.

Telehealth / Telemed
Do you offer telehealth appointments?

Yes. Most services are available via secure, HIPAA-compliant video sessions, accessible from any modern mobile device or computer. 

How do I access my telehealth session?

You’ll receive a link before your session. Just click it at your scheduled time using a phone, tablet, or computer.

Is telehealth as effective as in-person care?

For many clients, yes. It offers convenience and accessibility without sacrificing quality.

Do I need any special technology?

Just a stable internet connection and a device with a camera and microphone. Let us know if you need tech support.

New Patient Questions
What should I expect in my first appointment?

Your provider will review your history, explore current concerns, and work with you to build a treatment plan.

What paperwork or documents do I need to provide?

Photo ID, insurance card, any past mental health or medical records you’d like us to review, and completed intake forms (available online).

Do you offer any orientation or onboarding resources?

Yes. New patients receive a welcome email with helpful info, and our front desk is available to guide you through your first few visits.

Ongoing Care
How often will I have appointments?

That depends on your needs and treatment plan. Some clients come weekly, others monthly. We’ll decide together what’s best.

Can I change providers if I feel it’s not the right fit?

Of course. Your comfort matters. Let us know and we’ll support a smooth transition.

How do I contact my provider between sessions?

Non-urgent messages can be sent through our secure portal or by calling the office. For emergencies, call 911 or visit the nearest ER.

Insurance and Payment
Do you accept insurance? Which providers?

Yes. We are in-network with most major plans including Aetna, BCBS, Cigna, UHC, and others. Visit our website or call for the most up-to-date list.

What if I don’t have insurance?

We offer self-pay options and a sliding scale for those who qualify. Don’t hesitate to ask—we want care to be accessible.

Do you offer self-pay or sliding scale options?

Yes. Self-pay rates are competitive and based on provider type. Sliding scale is available based on income and family size.

What payment methods do you accept?

Credit/debit cards, HSA/FSA cards, and some digital payment methods. Payment is due at time of service.

Prescriptions and Medication
How are prescriptions handled?

Prescriptions are sent electronically to your preferred pharmacy after your provider visit.

What if I need a refill?

Refill requests can be submitted through our patient portal or by calling our office. Please allow 48 business hours for processing.

Can I discuss concerns about side effects or changes in medication?

Absolutely. We encourage open conversations about medication response and side effects. Don’t wait—reach out.

Privacy and Confidentiality
How do you protect my privacy?

We follow HIPAA regulations and safeguard all client information. Privacy is core to our values.

Is my information secure during telehealth sessions?

Yes. We use encrypted, HIPAA-compliant platforms for virtual care.

Can I access my medical records?

Yes. You can request records through our office or securely access them via our patient portal.

Crisis and Emergencies
What should I do in a mental health crisis?

Call 911 or the Suicide & Crisis Lifeline at 988. We are not a crisis center and cannot guarantee immediate response outside of session times.

Do you offer emergency or after-hours support?

We do not offer 24/7 services, but we can connect you with local crisis resources. Some providers may offer limited after-hours communication.

Can I walk in without an appointment?

We are appointment-based and do not offer walk-in services. Please call or message us first so we can best support you.
